Medical Record Requests
The Health Information Management department maintains records of the care patients receive while they are at Palmdale Regional Medical Center. As required by state and federal law, these records are kept in strict confidence and are only released with proper authorization.
We offer the following options to allow patients to obtain medical records and/or radiology studies from Palmdale Regional Medical Center:

For the quickest access, retrieve your records directly through our patient portal, Health Records Online, which enables you to access your records on-demand at your convenience.
Submit your medical record request through our online medical correspondence system.

Providers may send a written request to our secure fax machine at 661-382-5499.
If you have any questions or would like additional information, please call us at 661-382-5048 and our staff would be happy to assist you. Please be advised that to protect our patient’s privacy and health information, we do not accept telephone requests and we do not disclose any information over the telephone.
Processing and copy fees may be applicable to your request. Costs will vary depending on the type of request. There are no fees associated with the patient portal, Health Records Online.
